Community Options Program
The Community Options Program at Interaction Disability Services aims to provide flexible service options to individuals with an intellectual disability and their carer/s.
Community Options is a case management service for clients whose needs cannot be met by mainstream Home and Community Care (HACC) services.
The Community Options approach begins with a holistic and comprehensive assessment of the individual client’s needs. The Coordinator works with the client/carer to develop a care plan that may involve HACC service providers, private agencies and family.

Our aim at Interaction Disability Services Community Options Program is to help clients, and their carers to remain as a family unit.
To do this we offer:
a needs assessment
care plan
service coordination
monitoring of services
service review
advocacy on your behalf
the use of brokerage (purchase of services) to achieve the client’s individual goals.
 
Some of the services a Coordinator may be able to assist you to access include:
homecare
community transport
respite services
recreation services
home maintenance and modifications
support group
physiotherapy
occupational therapy
podiatry
Community Options is focused on keeping the family unit together. We provide a Coordinator who will:
in consultation with the client/ carer develop an individual care plan
assist the client and their family to access the services they need
negotiate with service providers on the client’s behalf
provide individual case management
review the care plan as required to meet any changing needs.
To be eligible for the Community Options Program you need to:
be aged 18 years or over
have moderate to severe disability
have complex care needs
be unable to access other services
be at risk of family break down
live at home with a carer/s
live in the Local Government Areas of Baulkham Hills, Blacktown, Holroyd, Parramatta or Penrith.
